NEW AND RETURNING ENROLLMENTS:
Required For Enrollment and Change of Address:
1. Child’s Birth Certificate
2. Child’s Shot Record
3. Child’s Physical (Kindergarten only)
4. Two proofs of residency in your name from any of the following:
- BILL OF SALE FOR HOME
- PROPERTY TAX DOCUMENTATION
- CURRENT UTILITY BILL, WATER BILL, PHONE BILL, ETC.
- DHHS, SSI, FOC, ETC. DOCUMENTATION
- AUTO REGISTRATION
- DRIVERS LICENSE OR VOTERS REGISTRATION (would only count as one)
The homeowner or renter must provide two proofs of residence and complete the residency affidavit, which must be notarized.
Proper paperwork must be provided in order to enroll a child if a birth certificate cannot be provided. If there are court ordered custody arrangements in place, they must be provided in order for the school to enforce them.
If your child has been receiving any Special Education services, please bring the most recent copy of your child's IEP with you to register. You will also be required to go to the Special Education Department after registering to ensure proper placement for your child.
If your child is 5 years old by 9-1-2020 they are eligible for kindergarten. If the birthday is between 9-2-20 and 12-1-20 they may register with a kindergarten waiver.
