Pre-
registration
New Residents, Relocating Residents, Kindergarteners, or
Parochial Students Returning to the Taylor School District
New residents to the Taylor School District, residents who
have moved within the district, or parochial students returning to
the district must preregister at the Pupil Accounting Office at 9551
Westlake (in the Board Annex) between 8:00 A.M. and 3:00 P.M. The
Pupil Accounting Office is open for registration throughout the
summer.
The following items are needed to complete registration:
1. TWO of the twelve current documents listed
below to prove residency: mortgage, rental receipt, lease (must have
landlord's name, address and phone number), bill of sale for place
of residence, auto registration, driver's license or voter
registration, water, phone, gas, or electric bill (or deposit
receipt), or social services papers with address.
2. Court placed documents, if applicable.
3. Birth certificate.
4. Immunization record.
5. Copy of most recent physical.

Emergency
Procedures
School closings due to inclement weather will be noted on
cable TV (in Taylor, Comcast Channel 22 or WideOpenWest (WOW)
Channel 15) and announced on TV Channels 2, 4, 7, and on radio
stations WJR, CKLW, WWJ, WEXL, WCXI, WDIV and WNIC.
Certain conditions (inclement weather, power failures, water main
breaks, etc.) sometimes necessitate early dismissals. We ask that
you instruct your children about what they should do if they arrive
home and you are away.
In the event of a tornado or other severe weather conditions,
the district has a program developed through the resources of the
Office of Civil Defense which provides maximum safety for the
children.

Health
Appraisals
(Physical Examinations)
Prior to entry into the Taylor School District, a student must
submit evidence of a completed physical examination.
If a student is transferring to Taylor Schools from another
school district, the parent MUST either:
a) Submit a copy of a previous physical, or
b) Submit a new health
appraisal.
The Michigan High School Athletic Association (MHSAA) mandates
that all students participating in interscholastic sports have an
annual physical examination on file.

GROWTH
AND
DEVELOPMENT
PROGRAMS
This year our school district will offer Growth and Development
programs at the following grade levels:
Grade 5: A school nurse consultant will present a
program focused on normal developmental changes associated with
puberty, as well as
special needs related to personal hygiene, nutrition and rest.
Grade 6: A school nurse consultant will
present a program focused on the physiological changes of puberty,
as well as the process of conception and pregnancy. The risks of
sexually transmitted diseases and the importance of sexual
abstinence are also discussed.
Grade 7: An abstinence based curriculum, "Healthy
Sexuality," will be part of an exploratory class. The lessons teach
skills necessary to resist sexual involvement and to develop a
strong social support system. Some of the activities include parent
involvement.

Emergency
Cards
On the first day of school, your child will be given an
emergency card to take home for completion.
It is important that both sides of the yellow card be
completed and returned the next day.
1. Correct phone numbers are vital. If numbers change during
the course of the year, inform the school office immediately.
2. List two persons (include phone numbers) in the immediate
area who are available during the school day in case the parent or
guardian cannot be reached. These persons must have transportation.
This is necessary in case of early dismissal, illness, or personal
emergency.

Medical
Requirements
Immunizations
Michigan law mandates immuniza-tion rules and regulations in
all Michigan school districts. According to state law, any child
who does not have proof of all required immuni-zations or a waiver
and who is entering the Taylor School District for the first
time, may register at the Pupil Accounting Office but will not be
allowed to enroll until proof of all required immunizations is
provided at the school building.
Please Note: Appointment cards cannot be
accepted as proof of immunizations!
Immunization requirements for children 4 to 6 years of age
entering elementary school include:
1. Four doses of diphtheria, tetanus, and pertussis (DTP)
vaccine. If the last dose was received before the child's 4th
birthday, an additional booster dose is needed.
2. Three doses of polio (OPV) vaccine. If the last dose was
received before the child's 4th birthday, an additional
booster is needed.
3. Two doses of measles, mumps, and rubella (MMR) vaccine at
or after twelve months of age, at least one month apart with at
least one of the doses on or after fifteen months of age.
4. Three doses of Hepatitis B vaccine. There must be 60 days
between doses two and three. There also must be a total of four
months between doses one and three.
5. One dose of varicella (chickenpox) vaccine or written
verification of having had chickenpox.
Immunization requirements for students over 7 years of age
entering the Taylor School District and all 6th graders currently
enrolled include:
1. Four doses of diphtheria, tetanus, and pertussis (DTP)
vaccine. A booster dose of DT is needed every ten years after
completing initial series.
2. Three doses of polio (OPV) vaccine.
3. Two doses of measles, mumps and rubella (MMR) vaccine at
or after twelve months of age, at least one month apart with at

4. Three doses of Hepatitis B vaccine. There must be 60 days

Notice To Parents:
Video & Photo Release Requested From
Parents
The Taylor School District from time to time takes, or allows
local media to take, still photos or videotape images of district
events; these pictures often include students in our schools.
If, for any reason, a parent or guardian of a Taylor
School District student does not want his or her child photographed
or videotaped, the parent or guardian must send a note stating the
specific request, including the name and grade of each child
involved, the name of the child's school, and the name of the
child's teacher (for elementary children) to the child's school.
Address the request to the attention of the building principal.

Medications
in the
Schools
If it is necessary for your child to take medication during
school hours, the following procedure is necessary, as required by
Michigan law.
1. The medication (either prescription or over-the-counter)
must be brought to school in the current prescription- labeled
container with the child's name, medication, dose, and time of
administration.
2. A medical authorization form must be submitted to the
school office each year.The forms are available in each school and
must be completed by the physician and parent. Drug dosage, time
interval for medication, and possible side effects or interactions
must be stated. Secondary students may self-administer their
medication; however, a doctor and a parent are still required to
sign an authorization form. Controlled substances may not be
self-administered.

AIDS Programs
It is mandated by the State of Michigan that AIDS education be
included in the curriculum. The Taylor School District is committed
to providing sound educational programs, stressing age
appropriateness and using guidance from the Michigan Department of
Education and the Michigan Department of Public Health. All program
material has also been reviewed and approved by our district's
Health Advisory Committee.
Grade 6:
A school nurse
consultant will present a lesson which includes a discussion on the
causes, transmission, and prevention of HIV/AIDS. Sexual abstinence
is stressed throughout the program. The video "HIV/AIDS Answers for
Young People" will also be shown.
Grade 7: An abstinence-only curriculum,
"Healthy Sexuality," will be a part of a exploratory class.
Information on sexually transmitted diseases, including HIV/AIDS,
will be presented by a teacher certified in sexuality education.
Grade 9: An abstinence-based curriculum, "Safer
Choices," will be presented by physical education teachers certified
in sexuality education. Information on HIV/AIDS and other sexually
transmitted diseases, pregnancy, and high-risk behaviors will be
presented. Abstinence will be stressed as the safest choice for
students, but accurate information about other methods of protection
also will be provided.
Grade 11: An abstinence-based lesson will be led by a
certified HIV/AIDS prevention instructor. The program will include
information on HIV/AIDS, high risk behaviors, and ways to protect
yourself from HIV.

Parents' Right to
Preview Programs
Excluded From
Instruction
All programs
related to AIDS, Growth and Development, and Hepatitis B instruction
are available for parent preview at the Professional Resource
Center, 11501 Beech Daly, Taylor. Call (734) 946-6584 for an
appointment.
Parents have the right to exclude their child from any of the
above programs. If you DO NOT want your child to participate,
please complete an exclusion letter and return it to the building
principal the first day of your child's attendance at the school.

Confidentiality
of Medical
Information
By writing health problems on the emergency card, you are
providing an annual update regarding your child's medical
conditions. This information is shared with staff on a "need to
know" basis. If you wish to keep medical information more
confidential, contact your child's school office to obtain a
Confidentiality Restricted Consent Form. Return the completed form
to the school office in a sealed envelope addressed to the School
Nurse Consultant.

Communicable
Diseases
Grades K-1: A school nurse consultant will present the
program "Scrubby Bear." This program is based on a Michigan
Department of Education unit on hand washing as a means to prevent
communicable disease.
